This privacy policy governs all information collected through any website owned and operated by HIPAOS, LLC.

HIPAOS' commitment to your privacy

We are obligated to make every reasonable effort to protect your medical information and your personal privacy from inadvertent disclosure. As described throughout this document, we take that obligation seriously, and have taken steps to meet or exceed the best practice standards for Internet security. For that reason, no account or personal information can be accessed or retrieved when a visitor browses the public (pre-login) areas of any HIPAOS web site.

It is our policy, except under the very narrow legal circumstances described in this paragraph, that no personal medical information contained in your account will be divulged to any other person. We will not sell, market or disclose your personal medical information to third parties or reveal other details about your health records without your permission. However, we reserve the right to disclose or report such personal information -- in limited circumstances -- where we believe in good faith that disclosure is required to cooperate with regulators or law enforcement authorities. We may need to share your private health information with our own affiliates and contractors, but only to the extent reasonably necessary to operate this website and provide related services. We do reserve the right to adjust this privacy statement in our sole and absolute discretion, without additional notice.

By confirming your agreement, you also pledge to hold harmless HIPAOS, LLC and/or its contractors under federal and state privacy laws for any inadvertent or deliberate online disclosure of personal information. You also acknowledge (through the content of this Legal Disclaimer and Privacy Statement, and by your continued use of any HIPAOS website) we have fulfilled our obligation to disclose how we will use your personal information.

Online security

We have taken many precautions to protect your information from loss, misuse or alteration. We operate secure networks protected by industry standard firewalls and password protection systems. Only authorized individuals have access to the information provided by our users.

We use Secured Socket Layer ("SSL") transmission encryption to protect transmission of personal information you enter into your online account and emergency response wallet file. SSL lets your browser send and receive information in an encrypted format. The information you store with us is encrypted before it is saved. That way, should any unauthorized user ever penetrate our security, your stored information can not be read.

Your browser's security icon, typically found at the bottom of your browser's window, reflects the security condition of the browser. The padlock icon indicates that your browser is communicating over a secured link.

Your responsibility

You acknowledge that we are only providing secure, private data storage plus blank wallet file templates for your personal use. Further, you agree you are solely responsible for the accuracy of information you enter into your account and which will be the source for data appearing on the personal medical wallet file you produce from that account.

You agree to hold us harmless for any misspellings, incorrect dosages, or other errors you make when entering your personal health information into our system. You agree HIPAOS, LLC, cannot be held responsible for your errors or negligent use of the service due to:

Errors of commission or omission while entering information;

Failure to maintain confidentiality of or sharing of passwords and/or access information leading to unauthorized access to accounts. This includes storing passwords (through functions such as "autocomplete") on any PC or device that may be collected and retransmitted (as in the case with "spyware.");

Failure to "Sign Off" when completing an online session, leaving a computer unattended during an online session, or storing identification or password information in a computer.

As a member, you have a responsibility to safeguard your personal and medical information. You can do so by following these simple guidelines:

Protect your sign-in information and password.

Use caution when disclosing your account access details to other persons. If someone contacts you, either by telephone or e-mail, says they represent HIPAOS and asks for your account information, do NOT give it to them. No one from HIPAOS, will ever contact you to ask for your personal information either by telephone or e-mail. We recommend you hang up and e-mail us at support@myemergencymedicalfile.com or telephone us directly at 321-274-1741 (between 9 AM and 5 PM, Monday through Friday, Eastern Time) to verify the call is legitimate.

Our promise to you

The HIPAOS data network is defined as the point from your modem connection on the Internet to the final system on which a given transaction is accessed and stored by one of our servers. We promise that this entire data network will be constantly protected by a system of firewalls and other technology, making it stronger and more secure than some online banking systems. We will use security coding techniques to protect against unauthorized access to personal records, ensure accuracy and integrity of communications and transactions, and protect member confidentiality.

You will need to use certain types of computers, obtain an Internet account, and use browser software before you can access HIPAOS' online services. The installation, maintenance, and operation of those items are your responsibility. We are not responsible for any errors or failures of your computer equipment or Internet connection software. HIPAOS' online services -- and our commitment to protect your privacy -- begin at the interface of your modem connection with the Internet.

YOUR CONTACT INFORMATION

A. How we store your personal contact information
When you provide us with your name and contact information, it is recorded in a secure database that is owned and managed by trusted HIPAOS contractors.

B. How we utilize your personal contact information
Your contact information will be utilized by HIPAOS or its business associates to contact you by mail or e-mail so we can provide information we think may be of particular interest to you.

We will contact you whenever . . .

You are entitled to free upgrades. We plan to change the format of our wallet files whenever better ideas come along. Whenever that occurs, we will tell you so you can print off a new, improved wallet file. We learn about new health issues we think are important to you (you will have the opportunity to opt out of these helpful mailings at your request).

If there has been no activity in your account for two years. We hope always to keep you as a member, but we will remove inactive accounts unless their owners respond to our e-mail inquiry.

You may also receive promotions or solicitations from us or third party associates. You can opt out of such mailings, either by clicking the unsubscribe link at the bottom of the e-mail itself, or by contacting us at support@myemergencymedicalfile.com. Regardless of whether you choose to unsubscribe to these mailings or not, you may continue to use the website and our services.

Your Password
Our computers will store your password as part of our security system. Should you ever need password help, simply click on the "Forgot My Password" link, and our computer will automatically send it to you via e-mail. If this does not work, or if you have changed e-mail providers, telephone us at 1-321-274-1741 (between 9 AM and 5 PM, Monday through Friday, Eastern Time).

How we track visitors to our web sites
An IP address is the unique number that identifies your computer from all others when you log onto the Internet. As you enter HIPAOS' site, our server records and monitors your IP address and uses it to develop aggregated statistical analysis. These analyses assist us in refining and adapting the content and design of our site. By accessing any HIPAOS web site you acknowledge and authorize such recording and monitoring of your computer's IP address.

The use of IP addresses also helps us prevent malicious Internet users from masquerading as authorized users. By interrogating your computer's IP address, HIPAOS seeks to protect your data from unauthorized users. However, HIPAOS hereby disclaims all liability should any unauthorized user access one of our sites with your user name, password and IP address.

How we use "Cookies"
A cookie is a piece of text or data that can be stored in the memory of your web browser. This data can only be retrieved by the server/domain that places the cookie.

HIPAOS employs cookies for the following purposes:

To enable automatic form validation/entry (if "remember me" is selected in site store).

To enable online polling (cookies allow us to prevent people from answering our surveys more than once).

To compile statistics concerning items offered online.

HIPAOS NEVER stores medical, financial or Credit Card data within cookies. Those who feel uncomfortable with cookie technology can disable cookies within their browser settings (if you are unsure how to do this, seek the advice of your particular browser's technical support professionals). Our web sites are designed to operate with or without cookies. However, some functionality will be removed when cookies are disabled, and HIPAOS shall not be responsible for lost or altered data resulting or related to such disabling of cookies.

The option to cancel mailings and e-mails
Upon registering ("opting in"), you may begin receiving communications from us. These messages will always contain unsubscribe instructions. If you would like to correct, update, add, or delete personal information, simply let us know by clicking here or telephoning 1-321-274-1741 Mondays through Fridays 9:00 AM to 5:00 PM Eastern Time.

Surveys
From time to time our site may request information from users via surveys. Participation is completely voluntary. Information requested may include demographic information (such as age and zip code). This information will be used for purposes of monitoring or improving the use and satisfaction of this site.

We will not record information from "e-mail this page to a friend"
If you use our referral service to inform a friend about our site, the friend's e-mail address is not saved, and we do not contact the friend again. We will ask for the friend's e-mail address for the sole purpose of sending this one-time e-mail.

YOUR MEDICAL INFORMATION

How we store your personal medical information
Storing your data allows our site to assemble your information so that you can then print your personal emergency medical information wallet file. When you log into our system and fill in the blanks to complete your personal emergency medical file, you are creating your own personal and private data storage account, much like an online bank account. Your encrypted information is stored on a server that is protected by state of the art security systems.

Your data is garbled by our encryption programs before being stored. What does that mean to you? Even if a hacker gets through our formidable safeguards, all they can retrieve will be gibberish.

How we utilize your personal medical information
The information you provide will be used to create your personal emergency medical file and wallet file, and for no other purpose.
